Post-exertional malaise (PEM) refers to the worsening of symptoms following physical, cognitive or emotional exertion.
It isn’t just a feeling of general tiredness, but can include severe fatigue, pain, cognitive dysfunction, sleep disruption, and more.
A delayed onset of symptoms
Post-exertional malaise is characterized by a delay in the onset of symptoms, which may occur from 12 to 48 hours after exertion and can last for days or even weeks.
This delay makes it extremely difficult to recognize when over‑exertion is occurring, which can lead to a debilitating cycle of relapsing and recovering.
Staying within your energy envelope
Pacing is a strategy that balances rest and exertion to minimize post-exertional malaise. It involves finding a comfortable range of energy expenditure that avoids both over‑exertion and under-exertion, maintaining an optimal level of activity over time.
